The Request for Service Application (RSA) is a web-based form and is the mechanism by which a registered NHLBI investigator applies for Gene Therapy Resource Program services.

» Review instructions for submitting an RSA before you start your electronic submission.

As the applicant investigator you must submit a Conflict of Interest Statement for Investigators (relevant to the study associated with the RSA).  The investigator and a representative of the investigator’s institution must complete, sign and submit the applicable Investigator and Institution Certifications and Agreement form
(this is not required for an RSA for Regulatory Support).

Select the appropriate forms from the list below and submit the completed forms to the GTRP Clinical Coordinating Center:

  (for all vector production RSAs and the Pharmacology/Toxicology RSA)

» Investigator and Institution Certifications and Agreement
  (for Clinical Trial Funding Assistance RSAs)
                          
» Conflict of Interest Statement
  (for Investigators)

Print, complete, and sign the selected forms.  (These forms are not completed on-line.)
Send the forms to the GTRP Clinical Coordinating Center.  Completed forms with original signatures must be received at the GTRP Clinical Coordinating Center before an approved RSA may be implemented.  Mail completed forms to:
